IMPORTANT NOTICE

New Patient Registration

We can register you as a patient if you live in Greater Huddersfield and are:

  • Homeless or in emergency accommodation
  • An asylum seeker

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms are available on the above "Registering Online" tab.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

New patients are initially taken on for 12 months. After that date their registration will be reviewed with a view to transferring to a regular practice.
